Erectile Dysfunction

Erectile Dysfunction (ED)
Erectile dysfunction (impotence) is the inability to get and keep an erection firm enough for sex. Erectile dysfunction, or ED, is the most common sex problem that men report to their doctor. The symptoms and causes of erectile dysfunction (ED). ED is often a symptom of another health problem or made worse by emotional problems.
How does Erection work?
An erection starts in your brain. Something you saw, felt, smelled, heard, or thought makes your nerves send chemical messages to the blood vessels in your penis. The arteries relax and open up to let more blood flow in; at the same time, the veins close up. Once blood is in the penis, pressure traps it within the corpora cavernosa. Your penis expands and holds the erection.
When your brain stops sending signals that indicate sexual arousal, the hormonal response ends. Your arteries go back to their normal state and your penis returns to a normal state.

Erectile Dysfunction Causes
Male sexual arousal is a complex process that involves the brain, hormones, emotions, nerves, muscles and blood vessels. Erectile dysfunction can result from a problem with any of these. Likewise, stress and mental health concerns can cause or worsen erectile dysfunction.
Sometimes a combination of physical and psychological issues causes erectile dysfunction. For instance, a minor physical condition that slows your sexual response might cause anxiety about maintaining an erection. The resulting anxiety can lead to or worsen erectile dysfunction.
Physical causes of erectile dysfunction
In many cases, erectile dysfunction is caused by something physical. Common causes include:
- Heart disease
- Clogged blood vessels (atherosclerosis)
- High cholesterol
- High blood pressure
- Diabetes
- Obesity
- Metabolic syndrome — a condition involving increased blood pressure, high insulin levels, body fat around the waist and high cholesterol
- Parkinson’s disease
- Multiple sclerosis
- Certain prescription medications
- Tobacco use
- Peyronie’s disease — development of scar tissue inside the penis
- Alcoholism and other forms of substance abuse
- Sleep disorders
- Treatments for prostate cancer or enlarged prostate
- Surgeries or injuries that affect the pelvic area or spinal cord
- Low testosterone
Psychological causes of erectile dysfunction
The brain plays a key role in triggering a series of physical events that cause an erection, starting with feelings of sexual excitement. Several things can interfere with sexual feelings and cause or worsen erectile dysfunction. These include:
- Depression, anxiety or other mental health conditions
- Stress
- Relationship problems due to stress, poor communication or other concerns

- whether you take drugs
- whether you are currently taking any medication
- your previous and current sexual relationships

Erectile Dysfunction (ED) Treatment
If you think you have ED, a good first step is to talk with your doctor. The treatment you need will depend on what’s causing it.
Choosing an ED treatment is a personal decision. However, you also may benefit from talking with your partner about which treatment is best for you as a couple.
The treatment for ED starts with taking care of your heart and vascular health. Your doctor may point out ‘risk factors that can be changed or improved.
You may be asked to change certain food habits, stop smoking, increase workouts or stop using drugs or alcohol. You may be offered alternatives to the drugs you take. (Never stop or change prescription drugs without first talking to your health care provider.)
Your health doctor may also suggest treating emotional problems. These could stem from relationship conflicts, life’s stressors, depression or anxiety from past problems with ED (performance anxiety).
The treatments below are available to treat ED directly.
ED Treatments
Non-invasive treatments are often tried first. Most of the best-known treatments for ED work well and are safe. Still, it helps to ask your doctor about side effects that could result from each option:
- Oral drugs or pills known (Viagra, Cialis, Levitra, Stendra)
- Testosterone Therapy (when low testosterone is detected in blood testing)
- Penile Injections (ICI, intracavernosal Alprostadil)
- Intraurethral medication (IU, Alprostadil)
- Vacuum Erection Devices
- Penile Implants
- Surgery to bypass penile artery damage for some younger men with a history of severe pelvic trauma. Penile vascular surgery is not recommended for older men with hardened arteries.
Each type has its own pros and cons. Discuss your options with your doctor to determine the best treatment for you.
Seeking treatment?
ED can be caused by underlying conditions that should be diagnosed and treated. You can start with your primary care doctor, but keep in mind that they may refer you to a sexologist. These specialists are trained to treat the urinary tract and the male reproductive system.
It’s important to communicate openly. Tell your doctor if you have other health issues or take medications. Explain how long you’ve had symptoms of ED and how it’s affecting your quality of life.
There’s a lot of information about ED online and no shortage of claims for quick fixes. Talk to your doctor before trying anything on your own. Those claims might be scams that won’t help your ED and may have the potential to harm your health.
Still, feel free to discuss any treatment options you’re curious about with your doctor. They can help you figure out what’s safe and effective, and what’s not.
